Summary: The universal joints at each end of the steering shaft are attached to the shaft with a key and set screw. The set screws may loosen and allow the key to fall out of place.
Consequence: If the key falls out, the universal joint would spin on the shaft, resulting in loss of steering and a possible crash.
Remedy: Modify steering linkage to prevent set screw from loosening and key from coming out of key way.
