- A required weld in the air ride suspension frame attaching area was omitted causing suspension and adaptor channels to be overloaded.
- Consequence
- Adaptor channels and suspension cross members could crack and fail with hanger and axle assembly detachment; this could result in a vehicle crash and/or a catastrophic axle detachment.
- Remedy
- Inspection and weld repair, if necessary, to attach the bogie subframe cross members to the bogie adaptor channel.
